What GPTs and Gems are

ChatGPT has GPTs; Gemini has Gems. Put simply, these are

a way to build an AI that is your own

Set the role, output format, voice, structure and cautions in advance, and it will produce output in a consistent shape without you writing the instruction from scratch each time.

Think of it as saving a form

GPTs and Gems are close to

saving the form of a piece of work

Three examples of forms worth saving:

Example 1: a patient handout GPT

  • no jargon
  • a register suited to older adults
  • mostly bullets
  • always three home exercises
  • always a "who to contact if you are worried" box at the end

Example 2: a case presentation GPT

  • ordered as background / assessment / intervention / outcome / discussion
  • one message per slide
  • always give units alongside figures and charts
  • adjust length to the time available

Example 3: an internal report GPT

  • for a manager; concise; bullets
  • readable in five minutes
  • ordered conclusion → evidence → proposal
  • plain words in preference to jargon

Build these once, and thereafter you hand over the material and get the same format back.

You stop starting from zero

In ordinary work, a surprising share of time goes not into thinking but into

shaping things into the same format again

Adjusting voice, ordering sections, writing headings, softening wording. GPTs and Gems take that shaping off your hands.

They suit organisations too

Not only individuals. This works at organisational level — hospitals, facilities, clinics, schools. When you want a consistent case report format, internal document format or patient explanation format, treating a GPT or Gem as a shared asset is a powerful idea.

It becomes easier to keep organisational knowledge

This is not only about efficiency. Turn into a GPT

  • how a veteran explains things
  • the way this facility thinks
  • internal rules
  • teaching know-how

and you may be able to

keep an organisation's knowledge in a structured form

Tacit knowledge that used to pass by word of mouth becomes something you can store as structured knowledge.

Turning knowledge into forms matters in an age of AI

For a long time, the person who knew a great deal was strong. From here on, the question becomes

how you structure what you know

GPTs and Gems are a tool for turning knowledge into a form.

Start small

There is no need to build something ambitious first.

  • an email you send often
  • material you produce every time
  • a routine report
  • a self-introduction
  • an explanation for new staff

Small is fine. A single "explanation for new staff" GPT is already useful. From there, gradually turn the forms of your work, your organisation and your teaching into AI.

AI can become a second self

The interesting part is that as you build GPTs and Gems,

your own way of thinking gets put into words

Because you have to articulate

  • how you explain things
  • what you weight
  • in what order you think

So while it looks like you are building an AI, you are in fact organising your own thinking. That feeds straight into teaching, running an organisation, and sharing knowledge.

Purpose

To produce clear patient handouts that do not stoke anxiety.

Style

  • minimal jargon; always gloss a term if you use one
  • a gentle register suited to older adults
  • mostly bullets
  • fits on a single A4 page

Required sections

  1. a gentle explanation of the condition
  2. three home exercises (purpose, method, repetitions, cautions)
  3. what to do if pain appears
  4. a box for who to contact if worried

Behaviour

When the user enters patient information, produce the handout in the format above. If information is missing, ask for what you need as a list.

Paste this into the "Instructions" field of a GPT and you have your own patient handout assistant.
